Social Prescribing
Through Connection

A practical, homebased option for
older adults and carers

Inclusee is a notforprofit social prescribing service supporting older Australians and carers who are lonely, socially isolated, or finding it hard to leave home.

Through our safe, welcoming online community, we provide structured social connection that complements clinical care and supports one of the most important determinants of health: social connection.

Why Social Prescribing Matters

Loneliness and social isolation are strongly linked to poorer physical and mental health, increased healthcare use, and reduced quality of life for older adults.

Health and community professionals are often the first to notice when someone:

  • Presents frequently with nonspecific concerns
  • Withdraws from activities or social contact
  • Experiences bereavement, transition, or loss of independence
  • Struggles to leave home due to mobility, transport, or caring responsibilities

Social prescribing offers a practical response — connecting people to meaningful, nonmedical supports that improve wellbeing and engagement.

Why Inclusee?

Inclusee is designed specifically for people who can’t easily access traditional, inperson community programs.

Inclusee offers:

  • Online access from home (no travel required)
  • Guided digital support for people with low confidence using technology
  • Structured social connection, not adhoc checkins
  • Interest based clubs and peer groups that build routine and purpose
  • Specialised programs for carers and groups at higher risk of isolation

Our approach supports ongoing participation, not just shortterm engagement.

Who Inclusee Is Ideal For

Inclusee may be appropriate for patients or clients who are:

  • Older adults living alone or socially withdrawn
  • Limited in mobility or fearful of leaving home
  • Experiencing loneliness, boredom, grief or life transitions
  • Carers experiencing stress, fatigue or isolation
  • Living rurally or without reliable transport
  • Willing to engage online with support

If social connection is impacting wellbeing — Inclusee is worth considering.

What Participants Experience

After referral, participants are supported to:

  • Join a welcoming online community
  • Receive help setting up and using technology if needed
  • Participate in regular online clubs (art therapy, e-safety learning, morning meditation, singing club, book club, carers haven, and more)
  • Connect with peers through shared interests
  • Build confidence, routine and meaningful relationships over time

Programs are delivered in a safe, inclusive environment and supported by trained staff and volunteers.

Using Inclusee in Practice

Identifying unmet social needs

Simple questions can help uncover isolation:

  • “How connected do you feel day to day?”
  • “What does a typical week look like for you?”
  • “What gets in the way of staying connected?”
  • “What activities did you used to enjoy that you do less of now?”
  • “What things do you enjoy outside of your role of caring for your loved one?”
